What can insomnia look like?
Symptoms may include taking longer than desired to fall asleep, waking repeatedly, having difficulty returning to sleep, or waking earlier than intended. Daytime effects can include fatigue, irritability, reduced concentration, low motivation, or distress about future sleep.
Sleepiness, fatigue, and nonrestorative sleep can also occur for other reasons. An evaluation considers sleep opportunity, schedule, health history, medications, substances, mental health, and symptoms of other sleep conditions.
How is insomnia evaluated?
The clinician may review a sleep diary, sleep timing, nighttime patterns, daytime function, previous treatments, and factors that began or perpetuate the problem. A sleep study is not routinely required to diagnose insomnia. Testing may be recommended when sleep apnea or another condition is suspected.
What treatment may include
CBT-I may address sleep scheduling, time awake in bed, conditioned arousal, and sleep-related thoughts. Recommendations are individualized; not every patient receives the same schedule or exercises. Progress may include less time awake, reduced distress, and greater participation during the day.
Is medication part of treatment?
Medication may be considered after an appropriate clinical assessment. Benefits, risks, interactions, prior response, alternatives, and patient preferences should be discussed. A website should not promise a sleep medication or imply that medication is always short-term or always necessary.
