An initial psychiatric evaluation is an opportunity to discuss symptoms, treatment history, health information, and goals in enough detail to develop informed recommendations. It is not simply a brief medication check or a questionnaire.
Helyx Health provides psychiatric evaluations for adults in Palo Alto and surrounding communities.
Preparing for the appointment
If available, bring or upload an accurate list of medications and supplements, previous psychiatric records, relevant laboratory or medical information, and the names of current clinicians. Consider noting when symptoms began, how they have changed, and what you most want help with.
Patients transferring medication management should provide details about current doses, refill timing, previous prescribers, and past medication trials whenever possible.
Topics discussed during the evaluation
The clinician may ask about mood, anxiety, trauma, attention, sleep, energy, appetite, thought content, substance use, medical conditions, family history, relationships, work, and daily functioning. Questions about safety, suicidal thoughts, self-harm, mania, hallucinations, or other serious symptoms are standard parts of responsible assessment.
Open and accurate information helps the clinician identify risks, potential diagnoses, and reasonable treatment options.
Diagnostic considerations
Psychiatric symptoms can overlap. Concentration problems, for example, may be related to anxiety, depression, ADHD, trauma, sleep disruption, substance use, medication effects, or a medical condition. Diagnosis may require additional history, records, standardized measures, medical evaluation, or observation over time.
Treatment recommendations
Recommendations may include medication, psychotherapy, changes to an existing regimen, coordination with another clinician, further assessment, or referral to another level of care. Medication is not guaranteed at an initial appointment.
The clinician should explain the reasoning, potential benefits and risks, alternatives, and next steps. Patients are encouraged to ask questions.
Follow-up care
When medication management continues, follow-up appointments review symptoms, functioning, side effects, adherence, and safety. The timing and length of visits depend on the clinician and treatment plan and should be confirmed during scheduling.
